AHRQ Survey Tool
The North Carolina Center for Hospital Quality and Patient Safety has contracted with the Georgia Hospital Association for use of their Culture of Patient Safety CoPS survey tool, a Web-based version of the AHRQ survey that is both easy to setup and take.
