Small Rural Hospital Transition Project to Value-based Care and Alternative Payment Models

The Small Rural Hospital Transition Project (SRHT) is designed to assist small rural hospitals in transitioning to value-based care and Alternative Payment Models (APM) such as patient-centered medical homes and hospital shared saving plans, as well as prepare hospitals for population health management.The SRHT Project application period opens Wednesday, Sept. 26 and closes on Wednesday, Oct. 24preview of the 2018 application is now available. Leaders are encouraged to download the SRHT Application Questions and the Self-Assessment Questions to begin work on the application in advance of the online release date. Refer to the ‘Helpful Hints’ to submit a successful application. Contact Rhonda Barcus or Bethany Adams with questions. Progress and outcomes for SRHT projects are captured at six and twelve month post project. Hospitals participating in 2017 are currently reporting positive outcomes. Their successes, implementation of the projects and the outcomes can be accessed through Hospital Spotlights.
