The North Carolina Population Health Collaborative is comprised of representatives of existing North Carolina Accountable Care Organizations (ACOs) and aspiring ACO participants and partners. The collaborative provides an opportunity for emerging and developing ACOs to share experiences and ideas about transitioning to accountable, value-driven, healthcare delivery systems, and to identify and discuss possible solutions for facilitating that transition.
The collaborative met March 7, 2019 and the theme was “Impacting Health Through Accountable Care Communities.” Tatyana Kelly, VP of Planning/Strategy & Member Services at NCHA, facilitated a panel entitled “Leveraging Community Initiatives to Build Accountable Care Communities.”
