New Platform to Connect North Carolinians with the Community Health Resources

The Foundation for Health Leadership & Innovation (FHLI) is working with NCCARE360 to build the North Carolina Resource Platform—a new statewide tool to make it easier for providers, insurers and community-based organizations to connect people with the community resources they need to be healthy. The NC Resource Platform will be open at no cost to all North Carolinians, systems, payers, and providers. It will be a robust, integrated resource database, website, call center, and care coordination platform for clinicians, social workers, care coordinators, families and others to connect people to the community resources they need. More details are here and here. Unite Us, an outcome-focused technology company that builds coordinated care networks connecting health and social service providers together is helping with the project. Please complete the steps below to join NCCARE360 as a Network Partner:
  • Complete Partner Registration Form:  This step must be completed for your organization to join NCCARE360. This form requests information on your organization, programs available, and staff in order to set up organizational profiles in the Unite Us software and create user accounts. You can complete the form HERE.
  • Register for Software Training: Direct users of the software such as case managers, community health workers, referral coordinators, etc. are required to attend an in-person or virtual 2-hour training to receive log-in credentials. You can register yourself and your staff for upcoming virtual software trainings.
