Care Transitions

The NC Alliance for Effective Care Transitions (NC ACT) was organized in 2010 with funding from The Duke Endowment. The mission of NC ACT is to have stakeholders work together for optimal healthcare when North Carolinians transition from one healthcare setting to another. Stakeholders represent hospitals, long term care, assisted living, home health, hospice, mental health, case management, insurance plans, community care networks, patients and others meeting to coordinate efforts to improve care transitions across all settings.

NCACT helps connect care transition services and resources, identify best practices and needs, and educate the public, providers, and decision makers on person-centered quality services.

Operating partners include: NCHA, the NC Department of Aging and Adult Services, The Carolinas Center For Medical Excellence, Community Care of North Carolina and Carol Woods Retirement Community.
