Implementing Root Cause Analysis and Action (RCA2)

Join the The Quality Center Patient Safety Organization and the North Carolina Healthcare Association on June 14 at NCHA in Cary for a workshop, “Implementing Root Cause Analysis and Action (RCA2).“

Have you read the National Patient Safety Foundation’s article, “RCA2: Improving Root Cause Analyses and Actions to Prevent Harm?” Curious about how to put the recommended methodology into action? Learn from Jessica Behrhorst, System Director of Quality & Patient Safety at Oshner Health System, who will review the foundational tools used in RCA2 and share her implementation experience.

Target Audience

Healthcare professionals involved with root cause analyses/event investigations, including quality and patient safety professionals, risk managers, medical staff and nursing leaders.

Learning Objectives

Upon completion of this course, you will be able to:

  • Define a Culture of Safety and the role it plays in a successful implementation of the RCA2 process.
  • Define the tools required for a successful implementation of RCA2 and engagement at all levels of the organization.
  • Discuss the barriers that may arise when creating a complex and sensitive process change such as RCA2 and how to overcome those barriers.

Jessica Behrhorst, MPH, CPHQ, CPHRM

Director of System Quality & Patient Safety at Ochsner Health System in New Orleans

Jessica oversees various projects involving patient safety, performance improvement, regulatory readiness, ambulatory nursing, and quality outcomes across Ochsner’s growing Health System. In her role, she has led the system-wide implementation of Root Cause Analysis and Action and has shared the organization’s learnings at various national meetings, including the National Patient Safety Foundation’s 2017 National Quality Forum.
