Care Access and Transformation Community

Led by the North Carolina Healthcare Foundation (NCHF), the Care Access and Transformation Community (CATCh) is a collaborative space for North Carolina Healthcare Association members to explore strategies and interventions that enhance care access and show potential to transform the overall care delivery system.  

In alignment with NCHF’s aim to remove structural and financial barriers to care and improve the overall health and well-being of North Carolinians, the CATCh will focus on non-medical drivers of health impacting individuals, communities, and populations across our state. The Community will have a particular focus on addressing the unique needs of populations with select chronic diseases such as congestive heart failure and diabetes. 

CATCh is open to all NCHA members and features quarterly webinars and networking calls directly informed by member needs. Through June 2026, the Community will focus on relevant topics such as regulatory compliance, communication and engagement, data-driven approaches to addressing non-medical drivers of health in select chronic disease states, and integrating patient-family voice to improve outcomes.
