Care Access and Transformation Community
Led by the North Carolina Healthcare Foundation (NCHF), the Care Access and Transformation Community (CATCh) brought together North Carolina Healthcare Association members to explore strategies and interventions that improve care access and support transformation across the healthcare delivery system.
Throughout the initiative, CATCh aligned with NCHF’s aim to remove structural and financial barriers to care and improve the overall health and well-being of North Carolinians. The program focused on non-medical drivers of health and their impact on individuals, communities, and populations across the state, with particular attention to those affected by chronic conditions such as congestive heart failure and diabetes.
Over the course of the program, members participated in quarterly webinars, networking calls, and peer learning experiences focused on topics including regulatory compliance, communication and engagement strategies, data-informed approaches to addressing non-medical drivers of health, and integrating patient and family voice to improve outcomes.
